Health Plan Choices
You have three health plan choices – Basic, Medium, or High.
All plans offer the advantage of the CHS PPO Network. Comprised of many of our attending physician staff and major local clinics, the CHS Network gives you access to the best providers in the County at the lowest out-of-pocket cost to you. If you prefer, you can use providers who are in the nation-wide Blue Cross PPO network or who don’t participate in a network, but you may pay more out of your pocket.

Prescription Drugs
All CHS health plans have the following prescription drug benefits:
- Retail – For up to a 30-day supply, you pay the greater of 30% or $5 per generic or $10 per brand name
- Mail Order – For up to a 90-day supply, you pay 30% of the discounted cost. Minimum and maximum copayments apply.
Dental Plan Choices
You have the choice of a Basic or High Plan. Both plans give you the option of using providers in the:
- Delta Dental Premier Network, made up of all Delta providers,
- Delta Dental Preferred Option (DPO/PPO), a smaller network offering lower cost care, or
- non-PPO providers, for which you’ll have the highest out of pocket costs.

Flexible Spending Account (FSA) Choices
You can use the Health Care FSA to reimburse up to $5,000 a year in eligible health care expenses tax-free. The Dependent Care FSA lets you save taxes on up to $5,000 a year in eligible dependent care expenses. Contributions are taken from each paycheck throughout the year before taxes are withheld.
Life and Accident Insurance
CHS provides you with basic life insurance of one times your salary and basic accidental death and dismemberment (AD&D) insurance of two times your salary. If you wish, you can purchase additional life insurance for yourself, life insurance for your dependents, and additional AD&D coverage for yourself and your family.
Disability Coverage Choices
CHS provides you with core long-term disability coverage that will replace 50% of your monthly salary after you have been disabled for 180 days. If you wish, you may purchase voluntary short-term disability coverage and/or voluntary long-term disability coverage that will replace more of your salary.
