COTTAGE HEALTH SYSTEM

RESIDENT PHYSICIAN BENEFITS

GOAL:
Cottage Health System (CHS) will offer a comprehensive benefits program to all eligible Resident Physicians.


POLICY:
Although Resident Physicians (Residents) are not employees of CHS, such individuals as a category are afforded the opportunity to participate in selected benefit programs offered by CHS. In all instances, the applicable CHS Summary Plan Description or insurance certificate governs coverage and coverage levels unless otherwise noted herein.

Eligibility:
Residents are eligible for the benefits outlined herein, beginning on the first day of their contract, provided they are actively at work on the first day of their contract. If not, benefits will begin on the first day they are actively at work.

Residents' benefits terminate on the last day of their contract. At that time, Residents may continue on the medical and dental plan through COBRA.

Medical Plan
CHS offers the High Medical Plan to Residents. The High Medical Plan includes prescription, vision and wellness programs. CHS pays the premium for single coverage. Resident's may add eligible dependents however the Resident is responsible for the applicable premium for that coverage. Exception: Mental health benefits for Residents are provided in-network at 100%. Residents must utilize the EAP services and obtain pre-certification for ongoing treatment. Otherwise, mental health benefits are provided at the non-network rate.

Residents may decline medical coverage provided they show proof of other medical coverage; however no pay credit is provided for doing so.

Dental Plan
CHS offers the High Dental Plan to Residents. CHS pays the premium for single coverage. Resident's may add eligible dependents however the Resident is responsible for the full applicable premium for that coverage.

Residents may decline dental coverage, however no pay credit will be provided for doing so.

Life Insurance
CHS provides life insurance at no cost to the Resident.

Long-Term Disability
CHS provides long term disability insurance at no cost to the Resident.

Miscellaneous Benefits
Residents are eligible for discounts (movie tickets, bus pass, amusement park, etc.). Residents are also invited to attend the annual picnic, and holiday party, and other employee events throughout the year.

Excluded Benefits
Unless stated in this policy, Residents are not eligible for the CHS Cash Balance Retirement Plan or any other CHS benefits.
