Please make sure to bring the following items with you on your admission day:

	•	Any advance directives about your health care that you have already completed.
	•	Comb, shampoo, cosmetics, toothbrush/toothpaste.
	•	Robe and sturdy slippers, and any item(s) that will make you feel more at home, such as a pillow or afghan.
	•	Your insurance identification or Medicare card and any required insurance forms.
